The 2-Way cleanout at 2967 Madeline St. required by OMC 13.08.400 was removed by a PSL contractor on 4-6-12. See Google Maps for a photo of what the cleanout looked like before it was removed. andes construction as part of a neighborhood sewer rehab project) If you scroll the Google map photo at the attached link you can see the cleanout next to the utility pole. The cleanout is no longer there.
